P08S08: Sexually transmitted infection (complications)

Bottom line: Information on sexually transmitted infections was used to persuade a patient to make a change (changing sexual practice). There was no information-related patient health outcome.

Level 1 outcome (situational relevance): On December 30, 2008, P08 did a search at work, by themselves, and after an encounter with a patient. They retrieved three information hits about sexually transmitted infections [STIs]. The reported search objectives were: to search in general or for curiosity, and to share information with a patient or caregiver. “She [the patient] is nineteen and she had had symptoms for about eight or nine months and couldn’t get access to care and then finally she could came to see me and then was wondering about complications [such as long-term complications of Chlamydia infection] and had lots of questions about it. […] [Specifically] she was concerned about the complications and infertility. […] We talked about it and then I said “if there is any information that I come up with, new or different, I’ll let you know.” […] I wanted to just look at what information was there. On STIs in general. […] And that was just to provide information, the most updated information for the patient.” According to P08, e-Therapeutics+ was the only source for information, and the found information was relevant.

Level 2 outcome (cognitive impact): Three hits were associated with a report of positive cognitive impact (see table). Regarding motivation to learn, reminder, confirmation, and reassurance, P08 stated: “I’m diagnosing things properly. […] I am familiar with differential diagnosis (regarding P08S08H02). […] I’m practicing with recommendations. […] I’m up-to-date I guess (regarding P08S08H03)

Retrieved information hit(s):

2) e-Therapeutics+ (CIRT): Therapeutics tab – chlamydia – Table 1: Differential diagnosis of vaginal discharge (P08S08H02)

3) e-Therapeutics+ (CIRT): Therapeutics tab – chlamydia – Whole chapter on Sexually transmitted infections (P08S08H03)

Level 3 outcome (information use): Information on sexually transmitted infections was retrieved, and used to persuade the patient to make changes (information used as presented in e-Therapeutics+). “I was able to provide additional information [to the patient]. […] I did [call her], yeah. […] When we are looking at complications, they’re looking for her to change her lifestyle practice (regarding P08S08H01)

Level 4 outcome (patient health): There was no clear relationship between the information use and patient health outcomes.
