P04S02: Migraine (same medication)

 

Bottom line: Information on migraine was used to maintain the management of the patient (medication). There were no information-related patient health outcomes.

 

Level 1 outcome (situational relevance): On July 11, 2008, P04 did a search at work, by themselves, and before an encounter with a patient. They retrieved one information hit about triptans and migraine. The reported search objectives were: to address a clinical question, to look up something they forgot, and to share information with a patient or caregiver. “[The patient was] a woman in her early 20s. […] I was reviewing my schedule, and it said that she was coming in for headache management and I know she is a young woman who has migraines so I just wanted to review what the latest information was on managing migraines, and prophylactic medication. […] Once again, it's all information that I know, it's just good to review it. It's not something I do frequently, migraine treatments, so if I'm not sure, I always like to look it up again. […] That was my intent, to share information with her [the patient] and review what she’s already been on and tried, and other options.” According to P04, e-Therapeutics+ was the only source for information, and the found information was relevant.

 

Level 2 outcome (cognitive impact): One hit was associated with a report of positive cognitive impact (see table). Regarding motivation, confirmation, reassurance, and reminder, P04 stated: “When I read something it always leads to something that I forgot that I had known, or it leads me to looking up one of the references which is a little bit different for everyone but it always triggers me to wanting to learn more about it. […] My first line would be using the NSAIDs, it’s listed there, and my second line, it was, the information was congruent with what my plan would have been, had I not consulted. […] [It was] just a good review.”

Level 3 outcome (information use): Information on triptans and migraine was retrieved, and used to maintain (be more certain about) the management of the patient (information used as presented in e-Therapeutics+). “She [the patient] didn't actually come in with a concern about migraines; it was a mistake in my schedule […]. She was actually doing well with the Naproxen that I had her on from the past. […] My management was justified, because I didn't need to change anything, she was doing well when she came in.”

 

Level 4 outcome (patient health): Without this information, P04’s management of the patient would have been the same. There were no clear relationships between the use of information and expected patient health outcomes.
