P02S08: Asthma in children (drug dosing and education)

 

Bottom line: Information on asthma was used to maintain the management of a patient (drug dosing and patient education). There were no information-related patient health outcomes.

 

Level 1 outcome (situational relevance): On September 15, 2008, P02 did a search at work in the emergency department, by themselves, and during an encounter with a patient. P02 searched for information outside of the examination room, and went back in with the drug dosage. They retrieved one information hit about asthma and Flovent. The reported search objectives were: to look up something they forgot, and to share information with a patient and her family. “She was a patient, seven year old girl, with known asthma, who was having an exacerbation and we were just reviewing her medications. And I’m not sure she was using the Flovent as she should have been so we needed to address that as part of her management. She was with her mom. […] I wanted to make sure I wasn’t giving her a too high dosage, just rechecking. […] And so to discuss with mom about proper dosing and use of Flovent.” According to P02, e-Therapeutics+ was the only source for information, and the found information was relevant.

 

Level 2 outcome (cognitive impact): One hit was associated with a report of positive cognitive impact (see table). Regarding confirmation and reassurance, P02 stated: “[The information confirmed I was doing the right thing] because we were using a medicine that’s recommended to prevent asthma from getting worse.”

Level 3 outcome (information use): Information on asthma and Flovent was retrieved, and used to maintain (be more certain about) the management of the patient (information used as presented in e-Therapeutics+). “[We] discussed with mom what the dosing should be based on her age or sort of the range and then she agreed that we should continue this medicine. So I believe for her as well we wrote a prescription for another inhaler for her to use. […] She was previously on the medicine and we were just updating the dose and that is the recommended medicine to be on to prevent deterioration.”

 

Level 4 outcome (patient health): Without this information hit, P02’s management of the patient would have been the same. There were no clear relationships between the use of information and expected patient health outcomes.
